2. ONLINE OPTION
If you complete the 4 forms below online we will print them for you and send them home with your child. You can come to Classical and see Mr. Rocco to have them Notarized or we can send them home with your child for you to have notarized on your own.
FORM A - IMPORTANT: Complete this form online, then come into school to sign with our notary (witness) or request a printed copy to bring to your own notary.
Form A - RIIL Assumption of Risk Form
Forms (B, C, D) - Just complete and sign online.
- B. Medical/Insurance/Parental Consent Form - This form has two sections. You must complete and sign section 1. Your physician must complete and sign section 2.
- C. Concussion Form - This form must be completed and signed by both parent/guardian and student.
- D. Classical High School Player's Guidelines - This form must be completed and signed by both parent/guardian and student.
